The majority of data we have on the extent of the problem come from surgical registries that do not account for ACL injuries that are not surgically repaired. The goal of this project was to calculate the total incidence of ACL rupture and its distribution by age and sex in Iceland regardless of treatment. Alongside the ACL injury it is common to injure other structures within the knee and this project aimed to describe the distribution of those concomitant injuries by age, sex, and anatomical location. Finally, potential factors associated with future knee health following an ACL rupture were assessed.

To calculate the incidence of ACL rupture and concomitant injuries all knee MRIs taken in Iceland between 2001 and 2011 were reviewed to identify those with damaged ACLs. These data were used alongside population data taken from Statistics Iceland to determine the incidence of ACL injury by sex and age as well as the distribution of concomitant injuries. All patients with a potential ACL tear were invited to complete an online questionnaire. Questions were posed regarding the circumstances, timing and treatment of their injury as well as the current health of their knee. These responses were used to understand the relationship between their current knee health and factors such as treatment, activity at time of injury, time since injury and concomitant damage.

The incidence of ACL rupture in Iceland was found to be 75 per 100,000 person-years. There were differences between the sexes with males showing a single peak at 23 years of age and women two peaks at 18 and 39. Concomitant injuries were seen in 87% of knees, the most common being a meniscal tear. The extent of concomitant injuries was strongly related to age at injury. A number of factors were associated with worse knee health including reinjury to the ACL and smoking. Conversely, injuring your ACL during sports was associated with better outcomes.

These data can be used to more efficiently direct health resources within the Icelandic healthcare system as well as to improve prognostic accuracy. Furthermore, they show the importance of preventing re-injury and not smoking.

Um doktorsefnið

Micah Kerwin Nicholls fæddist í Bretlandi 1975.
